Injury and Incident Reporting Procedure

1. Purpose

This procedure explains what coaches, team managers and volunteers should do when an injury, accident, near miss or concerning incident occurs during South Galway Stormers Basketball Club activity.

2. Immediate Response

  1. Stop the activity if necessary and make the area safe.

  2. Assess whether first aid or emergency services are required.

  3. Contact the parent/guardian for underage players as soon as practicable.

  4. Do not move a player with a suspected serious head, neck, spine or fracture injury unless necessary for immediate safety.

  5. Record what happened as soon as possible.

3. Emergency Situations

In an emergency, call 112 or 999. Provide venue address, access details, nature of injury, age of player and any known medical information. A responsible adult should stay with the injured player until parent/guardian or medical help arrives.

4. Concussion and Head Injury

Any suspected concussion or serious head injury should be treated cautiously. The player should be removed from play and should not return to training or matches the same day. Medical advice should be sought and return to play should follow appropriate medical and sport guidance.

5. Incident Report Form

An incident report should be completed for injuries, accidents, near misses, behavioural incidents, safeguarding concerns, facility hazards or any event that may later require follow-up. Completed general incident reports should be sent to the main club email / General Manager (sgstormersbc@gmail.com). Safeguarding concerns must be directed to the Designated Liaison Person (dlp.sgsbc@gmail.com) or Club Children's Officer (cco.sgsbc@gmail.com).

  • Field: Information to record

  • Date/time/location: When and where the incident occurred.

  • Person involved: Name, age group/team, parent contact where relevant.

  • Description: Factual account of what happened, avoiding assumptions.

  • Witnesses: Names and contact details if appropriate.

  • Action taken: First aid, parent contact, emergency services, removal from play, follow-up.

  • Reporter: Name, role and signature.

6. Safeguarding Incidents

Where an incident involves child welfare, alleged abuse, bullying or inappropriate adult behaviour, it must also be reported under the safeguarding, anti-bullying or complaints procedure as appropriate and directed to the Designated Liaison Person (dlp.sgsbc@gmail.com) or Club Children's Officer (cco.sgsbc@gmail.com).

7. Insurance and Claims

Where an injury may require an insurance claim, the Club should check Basketball Ireland insurance/claim guidance and retain all relevant records.

8. Storage and Review

Incident forms should be stored securely. The Committee should periodically review incidents and near misses to identify facility, equipment, supervision or coaching improvements.
